OPTION 1: ANTHEM DENTAL & ANTHEM VISION:
- The state offers these plans through Anthem Dental/Anthem Vision (outlined in the 2026 Benefits Selection Guide).
- NOTE: There are NO employer contributions made toward the Anthem Dental/Anthem Vision.
- If you wish to enroll in the Anthem Dental or Anthem Vision, action is required NOW. Make those elections now at khris.ky.gov.
Qualifying Event: If you have a life changing event causing you to require a change in health insurance coverage, contact Fay Mann immediately as there are strict deadlines from the date of event to submit an application for change of coverage.

Living Well Promise: To receive the $40.00 a month discount on your health insurance premiums for 2027, you MUST complete the LivingWell Promise by Castlight by 7/1/2026.
